Referral Sources

Alacare provides homecare information for all referral sources.

This section features referral guidelines to aid in determining if a patient qualifies for homecare and information on how to complete an Alacare homecare referral.

Referrals can be completed using Alacare’s online form that, upon completion, can be sent directly to Alacare’s Admission Services.

How to Make a Referral

Alacare’s Admission Services Department is committed to providing patients with quality care and enhanced services. Patients’ home health needs can be arranged with just a phone call, and Alacare ensures a smooth transition to the home care environment for each patient.

Alacare has taken steps to provide excellent customer service by enhancing its communication efforts. Alacare follows special requests, physician preferences, guidelines and parameters upon receipt of referrals. An experienced home health nurse is available, upon request, to provide in-home coordination of services for patients who have complex needs or require complex procedures.

Nurses generally will provide patient evaluations within 24 hours of referrals if needed.

Alacare’s normal business hours are Monday through Friday from 8:00 am to 5:00 pm. However, an on-call nurse is available 24 hours a day.


The patient has the ultimate choice in choosing a home health provider. However, if a physician determines home care is necessary for the patient, any staff nurse in the institutional setting may forward relative patient information to Alacare to begin the patient’s home care admission process.

Phone/Fax/Internet Referral
After the physician has requested homecare for the patient, office staff may forward the referrals to Alacare via fax, phone or Internet.
